WHAT TO EXPECT AT YOUR FIRST OFFICE VISIT

First ND office visit: Please download and complete the ND intake form prior to your first office visit.  Whenever possible, bring in a copy of your most recent lab work from the previous  6 -12 months. The initial visit is comprehensive; we will review your medical history in detail and discuss your health concerns.

Dr. Sunhee will recommend additional lab tests if deemed necessary.  At the end of your visit, you will receive an individualized  action plan on how to address your specific health concerns.

First Acupuncture visit: Please download and complete the Acupuncture intake form prior to your first office visit.  Make sure to wear a loose clothing that can be easily adjusted to access upper and lower extremities for acupuncture treatment.  We also encourage patients to use a bathroom prior to starting acupuncture treatment session.

FEES AND POLICIES REGARDING NATUROPATHIC MEDICINE

Naturopathic Physician License varies by state.  Since Illinois does not have a scope of practice for Naturopathic Medicine, we cannot accept insurance as a form of payment. We can provide you with a receipt, upon request, that may be accepted by insurance plans with a FSA or HSA options. Please consult your insurance company regarding reimbursements.

Visit fees are due at the time of service.

If it is necessary to cancel your scheduled appointment, we recommend that you call or cancel online scheduling system at least 24 hours in advance.  Your early cancellation will give another person the possibility to have access to timely medical care.

Note that all testing and consultation are done to assist in creating an individualized treatment plan to reach your optimal wellness.

During Naturopathic Medicine visit, No medical diagnosis is made.

Please seek care from an Illinois licensed physician for a medical diagnosis.
